Family Forward Montana Business Participation Application

Please fill out the entire application form and make sure to upload all required documents and files. Once an application is submitted, it may take us up to 15 business days to contact you about your application status.

When completing the application to become an endorsed Family Forward MT Employer, you will be asked to submit files and/or language for the following: 

  • Family friendly policies 
  • Company logo 
  • Company website 

We recommend having this information prepared before starting the application, as you will be unable to save the application to finish at a later time

Policy Sharing

Waiver to Use and Share Business Policies

As a valued participant in the Family Forward Montana program, we appreciate your commitment to creating family-friendly environments and fostering thriving communities. In order to further support businesses like yours and promote best practices throughout our network, we kindly request your permission to use and share the business policies you have submitted.

By agreeing to this waiver, you grant Family Forward Montana/Zero to Five Montana the right to use and share your business policies with other program participants, as well as on our website and digital media platforms. This will allow us to promote and disseminate exemplary policies and strategies that can inspire and support other businesses on their journey to become more family-friendly.

We understand the importance of protecting your intellectual property and are committed to maintaining confidentiality and proper attribution. Any reference to your business or organization in sample policies will be removed before sharing the policy with other members. Any policies shared or published will be done so in a manner that respects your ownership and integrity as the creator, in accordance with applicable laws and guidelines.

By granting permission, you affirm that you have the authority to authorize the use and sharing of the business policies submitted, and that you hold the necessary rights to do so.

This waiver serves as a testament to your dedication to improving and building a supportive community for working families. Your contributions will help pave the way for sustainable and inclusive family-friendly practices throughout Montana.
